I came for a facial and was asked to complete a questionnaire. This was, I am told, a part of the usual procedures for Treatwell. My complaint is not about the salon nor treatment but about your request to disclose sensitive personal information. Intrusive requests for medical history, medication list are not relevant. As part of Information Governance data requested needs to be proportionate and minimally intrusive. In my view this is neither. Refused to complete. Unhappy!
