At time of booking, £50 deposit non-refundable if less than 48 hours’ notice given.
Medical consultation:
The client is expected to divulge all medical conditions past and present to minimise the risk of an adverse reaction.
The practitioner will discuss all possible side effects and treatment in the event of an adverse reaction.
Relevant consent forms to be signed by the client.
Data protection:
Medical information and photos taken from iPhone with up to date security settings that may be later stored in cloud that is password protected. Full McAfee security in place on PC and Laptop.
Other information:
For toxin treatment a review may be agreed at 2 weeks post treatment. This does mean a review and not necessarily a “Top up”. After 2 weeks the toxin will have expired and will need to be destroyed.
Consent to use photographs for promotional use or to discuss with other practitioners for clinical supervision purposes. Photos will be edited to minimise client identification and promote privacy.
The client understands that reactions can occur, and the treating practitioner should have the opportunity to rectify.
For Health and Safety reasons, only 1 chaperone allowed. No children under the age of 15 years old allowed on site due to availability of space.
Contactless is preferred and please do not be offended if bank notes are checked for counterfeits.
